If you would like to request copies of medical records or x-ray films, please call 288-4388, ext. 3342. If you already requested records, please allow at least 48 hours from the time you requested them until they are ready to be picked up.

Upon receipt of your invoice, include the bottom portion of the invoice along with your check for the balance due. We accpet all major credit cards and checks.
